Rodney Medical Form Reminder!!

Rodney Families,

All Scouts and participating adult, need to have a physical prior to arrival at camp. BSA is very particular about the medical forms that are used so ONLY use the one below. Deviation from these forms will delay the entire Troop's entry into camp. Parts A and B can be filled out by the parent while Part C has to be completed by a physician.

If your Scout has had a physical within one year of departure (July 22th) then all the physician's office has to do is fill and sign the Part C.
